Item Description Reviewed
I have reviewed the item description, condition notes, photographs, requested work, and quote for accuracy.
Pre-Existing Condition
I understand jewelry may contain hidden defects, prior repairs, cracks, wear, thin metal, loose stones, undisclosed treatments, or other conditions not fully visible at intake.
Gemstone Risk
I understand gemstones, especially client-supplied stones or stones with feathers, chips, fractures, inclusions, or treatments, may chip, crack, break, discolor, or become damaged during repair.
Heat, Laser & Repair Risk
I understand soldering, laser welding, polishing, tightening, stone setting, cleaning, rhodium plating, and related repair processes carry risk depending on the item condition and materials.
Additional Work
I understand some repairs may reveal additional damage. I authorize Vermont Gem Lab to proceed according to the written quote and any pre-authorized amount listed.
Authorization
I authorize Vermont Gem Lab to perform the requested repair work according to this repair intake and agreement.
